Next, fill out your information and the music title information on the order form.

PLEASE NOTE: KINDLY LIST YOUR TITLES ALPHABETICALLY AND INCLUDE THE DISC CATALOG NUMBER. LIST ACCORDING TO ARTIST. THIS WILL TO HELP EXPEDITE THE PRODUCTION AND DELIVERY OF YOUR ORDER. If the catalog number is not included, we will fill the title request with the disc that that the most sold track record from our library. There will be a charge if replacement for the title is requested. After completing the form, you can then mail or fax the written order to us.

Upon receipt, we will then review your order for titlecard availability and then contact you with the information for completing your request.

With ROWE jukeboxes being the most common jukebox available, our default titlecard for the PRE-PRINTED ROWE FORMAT is CD-1P.

Next, enter your information and the music title information in the Excel Spreadsheet Order File.
Please list the Artist with LAST NAME, FIRST NAME format. Groups, just list as is (ex. Steve Miller Band), CD Title, Catalogue Number (this will be the last 5 or 6 digits of the barcode found on back of the CD jewelcase). THIS WILL TO HELP EXPEDITE THE PRODUCTION AND DELIVERY OF YOUR ORDER. After completing the form, you can then email the file to us. Upon receipt, we will then review your order for titlecard availability and then contact you with the information for completing your request.

We ship your item(s) F.O.B. which means once it leaves our warehouse, it becomes yours!
We are not responsible for lost or damaged orders.
To insure your shipment from loss or damage, may we suggest you add an additional $1.75.
 
Note: Custom orders are required to be paid for in full. There are additional charges for art work or custom colors.
Because all orders on this site will be created especially for you when you order them, all transactions are final.
Please order correctly. For pre-printed CD titles, please provide the catalogue number for title accuracy. Without it, we will ship the title that has sold the most from our library over the years. If this is not correct, there will be a charge for creating and shipping the correct title.
There are no returns or refunds on orders placed.

We try to take the worry out of having prepaid for your order.
Generally, in-stock titlecards will be shipped within 24-36 hours after receipt of your prepaid order. All orders are shipped best way.

In the event the titlecards you have ordered are out of stock or discontinued, we will make sure a comparable titlecard or titlestrip is offered. When the titlecards return to stock, we will promptly ship your order to you. If you decide not to accept the comparable offering, we will make sure your funds are returned to you. We want everyone to enjoy a win-win situation and when that happens, it's more fun that way for everyone!

We do not ship C.O.D., so all orders placed must be prepaid. You can call us toll free at 1-800-772-3345 with your credit card number to place your order. You can also provide us with your credit card number when you send us your order.

If you are sending your order via mail, include your check or money order for the total amount due with your order request.
